Many families are facing a multitude of decisions when a love one passes.  In an attempt to assist you with the arrangements for your love one, the information below serves as a guide. 

Information to fill out the death certificate:

• Decease full legal name
• Occupation (if retired, what they did most of their life)
• Date of birth
• Place of birth
• Social Security Number
• Address of Residency
• Spouse name (Maiden name if female)
• Father's name
• Father's place of birth
• Mother's maiden name
• Mother's place of birth
• Place of disposition
• Discharge papers if veteran
• Educational background

You need to:  (we can assist you with the below listed items if you need our help)

• Contact you minister and establish time and place of service.  Also find out if a musician is available to you.
• Decide what cemetery you would like to use.
• For a veteran, have or get the DD214 for burial in order to obtain a flag and to bury in a national cemetery. (National Cemeteries are not open on weekends)

We will provide you with:

• The proper form and information to place the obituary in the newspaper.
• A general price list to assist you in purchasing services, casket and any other merchandise.
• An itemized contract and discuss payment arrangements.

Note:  We ask that you bring the clothing doing the arrangement or the day after the casket selection.  Clothing includes everything except shoes.  If desiring a wig, please present with clothes.  If a beautician is a desire, we can provide one or you may provide you own.
